HSN to host transitions of care forum

Health Sciences North, along with the North East CCAC and Bayshore Home Health, will host a public forum on transitions of care at the Steelworkers Hall Thursday, Oct. 30.

“The successful transition of patients from the hospital back into the community is one of the most challenging but urgent health-care issues we are facing in our city and northeastern Ontario,” said Dr. Denis Roy, Health Sciences North's president and CEO, in a release. “It is going to take a total community effort to deal with this issue, and that’s why this public forum is so valuable, so we can hear directly from the public on where the gaps are and how we can make the system better.”

The public forum will start at 6 p.m. and will feature a number of panelists including David Lamb, manager of policy for the Transformation Secretariat of the Ontario Ministry of Health and Long-Term Care; David McNeil, HSN’s vice-president of clinical programs; Frankie Vitone, senior director of care co-ordination for the North East Community Care Access Centre; and Carol Talevi, a patient advocate in Greater Sudbury.

Steve Paikin, host of TVO's The Agenda, and chancellor of Laurentian University, will moderate the forum.

“To work together effectively and provide true patient-centred care, we have torn down the silos that used to exist in the health-care system, integrated our efforts, and included the patient voice,” said Richard Joly, CEO of the North East Community Care Access Centre, in a release. “Our goal is to find the most appropriate care venue for each individual patient, whether that’s home, assisted living, a retirement home or long-term care facility.”
