Programs that deal with different chronic diseases at Health Sciences North will team up for an education tour across northeastern Ontario.

Representatives from Health Sciences North’s nephrology (kidney disease) program, chronic disease self-management program, and the complex centre for diabetes care program will be travelling together to different parts of northeastern Ontario for education sessions with health care workers and patients.

The team will visit Kapuskasing May 26 and 27, Kirkland Lake June 22 and 23, and New Liskeard June 24 and 25.

“We know that one chronic condition could lead to or affect another, for example diabetes and kidney disease. We wanted patients to have the opportunity to learn how these conditions interact so they can be better prepared to manage them,” said Stephanie Cranston, the clinical manager of Health Sciences North's nephrology program.

“We’re also including health care workers this year because they may not always have easy access to specialists in chronic disease management, and by educating them, it also helps them when they are dealing with patients.”
